Acceptable Forms of Authorization:
-
Federal Aviation Administration (FAA) Form 8610-1, "Mechanic's Application for Inspection Authorization".

NOTE 1:
Item 12 on the form, "REMARKS" block, must state,
"Expires 30 days from date of inspector's
signature."

NOTE 2:
Item 14 must be completed,
including inspector signature, endorsement
block checked, and office identification
provided. (Refer to Order 8080.6D,
Appendix 1, Figure 18, for sample
form.)

NOTE
3: After reviewing
Form 8610-1 for proper completion
and endorsement, proctors must retain
the form. The original 8610-1 must be destroyed by the test proctor
once the applicant has been issued an official test report.
The proctor must make a copy of the form and attach it to the applicable daily log.
(Refer to Order 8080.6D, par. 6-1.c.)
- Failed, passing or expired Airman Knowledge Test Report, provided
the applicant still has the ORIGINAL test report in his/her possession. (See
RETESTING explanation(s) below.)
- "Expired test/credit" letter issued by the Airmen
Certification Branch (in lieu of a duplicate Airman Knowledge Test Report).
Retesting:
- Requires a 90-day waiting period for retesting.
- Applicants taking retests AFTER FAILURE are required to submit
the applicable score report indicating failure to the testing center prior to
retesting. The original failed test report must be retained by the proctor
and attached to the applicable sign-in/out log. The latest test taken will
reflect the official score.
- Applicants retesting IN AN ATTEMPT TO ACHIEVE A HIGHER PASSING
SCORE may retake the same test for a better grade after 30 days. The latest
test taken will reflect the official score. Applicants are required to submit
the ORIGINAL applicable score report indicating previous passing score to the
testing center prior to testing. Testing center personnel must collect and
destroy this report prior to issuing the new test report.
