Acceptable Forms of Authorization
- Aviation Mechanic Tests:
-
Original
Federal Aviation Administration (FAA) Form 8610-2, Airman Certificate and/or
Rating Application.
- NOTE 1:
Proctor is required to check
for appropriate check blocks
filled (in upper left corner
of form). Those not applicable
will have a line drawn through
them. (Refer to Order 8080.6D,
Appendix 1, Figure 15, for example.)
If either or both the Airframe
and Powerplant boxes are checked
on the FAA Form 8610-2, along
with the 'Original Issuance'
box (and NOT the 'Added Rating'
box), this serves as authorization
for the Aviation Mechanic General
(AMG) exam. (If the 'Added Rating'
box is marked, this indicates
that the AMG exam is NOT required.)
- NOTE 2:
Proctor is required to ensure block
V is completed, including date,
original signature, and office
designator. (Refer to Order
8080.6D, Appendix 1, Figure
15, for sample form.)

NOTE 3:
The applicant must retain both original 8610-2 forms issued by the Flight Standards District Office (FSDO).
The proctor must make a copy of the form and attach it to the applicable daily log. (Refer to Order 8080.6D, par. 6-1.c.)

NOTE 4:
The Form 8610-2 for a graduate of a Part 147 School officially affiliated with a testing center does not require block V to be completed.
- A graduate of a Part 147 school,
officially affiliated with a testing
center, may take the knowledge test
upon presenting an appropriate graduation
certificate or certificate of completion
to the affiliated testing center.
A graduate's name must be on the
certified list received from the
Part 147 school prior to administering
the appropriate test(s).
Acceptable Forms of Authorization - DME:

Signed letter of acceptance from the FAA National Examiner Board. The original letter shall be destroyed by the test proctor after the
applicant has been issued an official test report. A copy of the letter may
be retained by the testing center.

NOTE: The NEB letter is good for two years from the date of the letter.
Acceptable Forms of Authorization (for ALL tests listed above):
- Failed, passing or expired Airman Knowledge Test Report, provided
the applicant still has the ORIGINAL test report in his/her possession. (See
RETESTING explanation(s) below.)
- "Expired test/credit" letter issued by the Airmen Certification Branch (in lieu of a
duplicate Airman Knowledge Test Report).
Retesting - Aviation Mechanic Tests:

Retests do not require a 30-day waiting period if the applicant
presents a signed statement from an airman holding the certificate & rating
sought by the applicant. This statement must certify that the airman has given
the applicant additional instruction in each of the subjects failed, and that
the airman considers the applicant ready for retesting. Requires a 30-day waiting
period for retesting if the applicant presents a failed test report without
a signed statement.
Retesting - DME:
- Requires a 30-day waiting period for retesting.
Retesting (for ALL tests listed above):
- Applicants taking retests AFTER FAILURE are required to submit
the applicable test report indicating failure to the testing center prior to
retesting. The original failed test report must be retained by the proctor
and attached to the applicable sign-in/out log. The latest test taken will
reflect the official score.
- Applicants retesting IN AN ATTEMPT TO ACHIEVE A HIGHER PASSING
SCORE may retake the same test for a better grade after 30 days. The latest
test taken will reflect the official score. Applicants are required to submit
the ORIGINAL applicable test report indicating previous passing score to the
testing center prior to testing. Testing center personnel must collect and
destroy this report prior to issuing the new test report.
